Medicare Card
Having your Medicare card handy helps us identify your enrollment in Original Medicare and your Part A and Part B effective dates. It can also help us identify your eligibility for Extra-Help.
Provider
List
Sharing your list of doctors helps us to confirm your doctor's participation in a given plan. Please include your Primary Care and Specialists names on your list.
Medication List
An updated list of your current medications and daily dosage will allow us to estimate your medication Tier and copays or coinsurance.
